Walker Physical Therapy accepts most insurance plans including PPO and Medicare.
We accept:
- Over 3000 PPOs
- Point of Service (POS)
- Medicare
- Motor Vehicle Insurance
- Workers' Compensation
We also offer a 30% discount to those paying cash out-of-pocket, high deductible insurance plans, and those opting out of their HMO's (See cash discounts below).
We also offer a payment plan program to those who are undergoing any financial hardships.

Private Insurance
Before arriving, call your insurance provider and ask for your deductible and co-pay. If Medicare, remember to also call your secondary insurance. You can download our insurance worksheet and use it as a helpful tool when calling the 800 number on the back of your insurance card to verify your benefits. Then, all you have to do is bring the insurance worksheet in on your first visit.
Workers Compensation
Please call us at (800) 916-6210 and speak to our workers compensation specialist and give them the following information:
- The name of the insurance carrier
- Your date of injury
- Adjuster's name and phone number
- Claim Number
As soon as we receive your information, our workers compensation specialist will work as fast as possible to authorization for your physical therapy so you can be pain free again.
Auto Insurance
Fill out our motor vehicle accident form and bring it with you on your first visit. We do not accept third party insurances but you can use your auto insurance or health insurance.
Attorney Lien
Please call us at (800) 916-6210 with your attorney contact information and fill out our lien form and bring it with you on your first visit. Our billing specialists will work out a plan for payments with your attorney.
